The end-of-life community matron team, established over the past two years, has been working with people and their carers to ensure they have access to appropriate services. They aim to enable people to be involved in decision-making and have informed choice regarding their care.

About the speakers
THE OXFORDSHIRE COMMUNITY MATRON TEAM

Pat Gordon, Trish Glynn, Sue Quantick and Claire Cador make up the community matron team covering six localities across Oxfordshire.

Community matrons are all senior nurses with experience in clinical leadership, coordination, advanced communication, and holistic assessments
and in using a patient-centred approach.
